Inspections: The Good, the Bad, and the Roaches

Souvlaki Fresh leads off our latest restaurant inspection report, with a temporary closure on June 27th due to roaches. The number of violations wasn’t that big, but sometimes the lack of quality outweighs the quantity.

Dead roaches on premises….5 dead roaches underneath POS by the reach in cooler in front line. 1 dead roach underneath shelf , 1 dead roach by the reach in freezer , 2 dead roaches by the mop sink in back prep room.
Pesticide/insecticide labeled for household use only present in establishment….RAID ants and roach killer spray in frontline underneath POS.
Potentially hazardous (time/temperature control for safety) food cooked/heated yesterday/on a previous day not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within a total of 6 hours. Food has been cooling overnight. ….slice gyro 46°/49° in glass door reach in cooler. See stop sale. Operator discarded food. **Corrective Action Taken**
Raw animal foods not properly separated from one another based upon minimum required cooking temperature in reach-in cooler….raw chicken above raw beef, raw fish in reach in cooler. **Corrected On-Site**
Roach activity present as evidenced by live roaches found….2 live roaches on floor mat by the reach in cooler in front line kitchen. 10 live roaches underneath 2 compartment sink , 1 live roach con the wall between reach in freezer and 3 compartment sink , 1 live roach on the floor in back prep room.

Souvlaki Fresh is in the plaza just south of Mizner Park. They had a much better inspection the next day, but July 12th inspectors again found dead roaches. A follow-up on the 13th showed zero violations.

The biggest number in our report goes to China Buffet & Grill in Deerfield Beach (Hillsboro west of Powerline). Inspectors noted 34 total violations on July 11th, including 13 marked high priority. This place is an ongoing inspection disaster, featured in our late May report with 40 violations and a 2-day closure, and then a week later they were closed again and were caught “dumpster diving” on video.
Get ready for this list of high priority violations:

  • All potentially hazardous (time/temperature control for safety) foods on buffet cold held at greater than 41 degrees Fahrenheit. Observed imitation crab salad (51°F); coleslaw (51°F); pasta salad (49°F); cut tomatoes salad (55°F); cut lettuce (58°F); on the salad buffet.
  • Cooked meats or poultry hot held at less than 135 degrees Fahrenheit or above. Observed cooked chicken 84° stored in the kitchen on the fry station at room temperature
  • Dented/rusted cans present. See stop sale. Observed 4 dented cans on the can rack. **Repeat Violation**
  • Employee failed to wash hands before changing gloves and/or putting on gloves to work with food. The cooks **Corrected On-Site** **Repeat Violation**
  • Employee handled soiled dishes or utensils and then handled clean dishes or utensils without washing hands. The dish washer. **Corrected On-Site**
  • Employee touching ready-to-eat food with their bare hands – food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed the dish washer grab cooked shrimp with bare hands from a container filled with cooked shrimp to eat the n the kitchen. **Corrected On-Site**
  • Employee washed hands with no soap. The sushi cook.
  • Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Observed cut honeydew 53°F and cut cantaloupe 53°F on the sushi and fruit side of the buffet. **Repeat Violation**
  • Potentially hazardous (time/temperature control for safety) food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. The sushi rice. Rice still hot cook just add the vinegar. **Corrected On-Site**
  • Raw animal foods not properly separated from one another based upon minimum required cooking temperature in walk-in cooler. Observed raw chicken stored over raw shrimp, raw blue crab, and raw ground pork in the walk-in cooler. **Corrected On-Site** **Repeat Violation**
  • Raw animal foods not properly separated from one another based upon minimum required cooking temperature in walk-in freezer – all products not commercially packaged. Observed raw chicken stored over raw beef in the walk-in freezer. **Corrected On-Site**
  • Single-use gloves not changed as needed after changing tasks or when damaged or soiled. The cooks **Corrected On-Site**
  • Stop Sale issued on potentially hazardous (time/temperature control for safety) food due to temperature abuse. Observed imitation crab salad (51°F); coleslaw (51°F); pasta salad (49°F); cut tomatoes salad (55°F); cut lettuce (58°F); on the salad buffet. Observed cut honeydew 53°F and cut cantaloupe 53°F on the sushi and fruit side of the buffet. **Repeat Violation**

Follow-up inspections on July 18th were much better.

The other big number in this report belongs to the Sushi Thai restaurant in the same plaza as Souvlaki Fresh, just south of Mizner Park. Inspectors found 27 total violations on July 8th, including 6 high priority. Mostly they were related to hand washing and storage of raw animal foods.
Others with more than 10 violations include: Duffy’s on St. Andrews (19 violations, 2 high priority); Chen’s Yokohama in Boca Lyons Plaza (16, 5); New Golden China on Palmetto near Powerline (14, 3); and Grille on Congress (13, 3).

On the good news side of things, the Subway on Powerline (near Olive Garden) had a perfect licensing inspection in this report. A few weeks later they had 4 violations, 2 high priority which is still pretty good.
Near-perfect inspections were reported for:

  • Bakery of France on SW 18th (1 violation, 1 high priority)
  • Four Seasons on Glades near Boca Rio (2, 0)
  • Einstein Bagels in Shadowood (2, 1)
  • Pizza Hut on 441 near SW 18th (3, 0)

Other notables in this report include:

  • Rotelli’s in its new location near Hobby Lobby (4, 1)
  • Casa Tequila on Glades across from the International Jewelry Exchange (4, 0)
  • Houston’s near Town Center (4, 3)
  • Sicilian Oven on St. Andrews (4, 0)
  • Tijuana Flats in Mission Bay (4, 0)
  • Padrino’s in Mission Bay (5, 0)
  • Dominic’s on Glades near Boca Rio (5, 1)
  • Puccini’s on Yamato (5, 1)
  • Dominic’s in Westwinds of Boca (9, 2)
  • Bamboo Wok on Glades near Lyons (9, 1)
  • First Watch on St. Andrews (10, 2)
  • Another Broken Egg in Royal Palm Place (10, 3)

One last note, China Cafe in Logger’s Run reopened with a so-so licensing inspection, 8 violations, 1 marked high priority.

Pad Thai restaurant in Margate was hit with multiple bad inspections last week, closing the place for at least two days. The worst of it involved roaches, roach excrement, and an egg sac. Oddly the previous inspection was just a few days earlier and while not great (12 violations), it wasn’t that bad either.
On the 23rd inspectors closed Pad Thai after finding 35 violations including 5 marked high priority:

  • Dented/rusted cans present. See stop sale , chaokoh coconut milk 2 cans.
  • Employee failed to wash hands before changing gloves and/or putting on gloves to work with food.
  • Observed Roach activity present as evidenced by 1 live roaches found crawling on prep table, 2 in Saran Wrap box, 6 plus on shelf where clean pots and pans are stored between hot water heater and ware washing machine, 2 crawling on wall by hot water heater, 1 nestled between pots hanging and walk over 3 compartment sink, 3 between wall and pipe by hand wash sink, 3 plus in wall holes over prep table, 2 crawling on floor by fryer, 2 in ziplock box over prep table, in kitchen, 1 egg sac in container with egg roll bags.
  • Raw animal food not properly separated from ready-to-eat food, raw beef with cooked pork same container operator separated food. **Corrected On-Site**
  • Roach excrement and/or droppings present, around outlets, storage shelf in kitchen where clean pots and pans are stored , on wall where there are holes and around ziplock box in kitchen.

The next day had two inspections. The first, with 27 violations, noted:

From follow-up inspection on 2016-06-24: At time of call back inspection observed 2 live roaches nestled between wall and pipe by hot water heater in kitchen, 2 live under drain board by washing machine, 1 under the dish machine, 1 live by wok, 2 live crawling on kitchen floor. … Observed roach excrement on wall over 3 compartment sink.

Then in another inspection later the same day:

At time of call back inspection observed 5 live roaches on base of ceiling 1 crawling on the wall by hot water heater. … At time of call back inspection roach excrement observed in kitchen.

The place passed inspection on June 27th, but still had 11 violations.
The only other inspection with double digits this week was for The Original Bagel Tree on Atlantic in Delray Beach, with 12 violations, 3 of them high priority.
On the good news side of things, Maoz Vegetarian (in Town Center), Outback Steakhouse on SW 18th, and the Coconut Cove Waterpark (South County Regional Park) each had only 1 violation.

The kosher Boca Grill in Somerset Shoppes had a reasonably good inspection with 5 violations, and Sal’s Italian on Palmetto Park Road had a still not bad 7 violations.

Boston Market Closed for Roaches

The Boston Market in Shadowood Square was closed on Thursday after restaurant inspectors found over a dozen live roaches in various locations including near the prep sink:

Roach activity present as evidenced by live roaches found. 1 on the floor by the prep sink located next to the walk in cooler, 3 behind the stainless steel wall behind the prep sink located next to the walk in cooler, 1 on the wall behind the dry storage shelving unit next to the prep sink, 4 between the wall and rolls of toilet paper stored on the dry storage shelving unit and approximately 5 on the boxes of aprons stored on the dry storage shelving unit.

While there were only 9 violations total, three were high priority and those were pretty bad. The other two had to do with milk not kept cold enough leading to a stop sale on those items.

An inspection the next day found zero violations and the store reopened. The location has had generally good inspections in the past.

Next worst in this week’s report is Sushi Masa, just east of Town Center, which had 17 violations with 6 marked high priority including a stop sale for temperature abuse:

  • Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly, Chlorine 0 ppm.
  • Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit, sprouts 52°, cooked rice 52°, operator stated TCS food in cooler overnight. No other TCS food in cooler at time of the inspection. See stop sale.
  • Raw animal foods not properly separated from each other in holding unit, raw chicken over raw shrimp in glass door reach in cooler. **Corrected On-Site**
  • Small flying insects in dining area.
  • Stop Sale issued on potentially hazardous (time/temperature control for safety) food due to temperature abuse.
  • Wiping cloth sanitizer solution exceeds the maximum concentration allowed, chlorine 200 ppm.

Their follow-up inspection two days later was nearly perfect with two minor violations. Sushi Masa had a less than spectacular inspection last fall, though they’ve also had some good inspections.

The biggest number in this week’s report was for Whistle Stop in East Boca. They had 18 total violations with three marked high priority. Whistle Stop also had a not-great inspection in October of last year.
The other somewhat big number this week was Pizza Chef Bistro, on the west side of 441 near the county line. Inspectors noted 15 violations, but the good news is that none of them were high priority.
Also, Chipotle in Westwinds of Boca had 11 violations with 1 marked high priority.
The best report this week comes from Carvel Ice Cream on Spanish River in East Boca, with a perfect inspection.

There was a near perfect licensing inspection for Burger House South on Federal Highway.

Cucina Fra Diavolo, an Italian restaurant in Delray Beach, was shut down for the third time in less than a year for roaches. Their inspection on Friday showed eight high priority violations including a stop sale for temperature abuse and roaches in various places including kitchen prep tables.

  • All potentially hazardous (time/temperature control for safety) foods in reach-in cooler cold held at greater than 41 degrees Fahrenheit. Cooked veal 47°-48°, cooked chicken 49° stored in 7-Up reach-in cooler since 2/17/16. Stop sale issued.
  • Dented/rusted cans present. See stop sale. Opened, rusted can of cocktail sauce on food prep table.
  • Hotel and Restaurant license is expired. **Repeat Violation**
  • Live, small flying insects in kitchen, food preparation area, or food storage area. Observed approximately 10 fruit flies in kitchen, approximately 10 fruit flies at urinal in men’s restroom.
  • Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Cut lettuce 57°-62°, cut tomatoes 58°-61° on buffet line.
  • Raw animal foods not properly separated from one another based upon minimum required cooking temperature in walk-in cooler. Raw chicken stored over raw salmon in walk-in cooler.
  • Roach activity present as evidenced by live roaches found. Observed 2 live roaches on food prep table in kitchen by cookline, approximately 15 live roaches in crevice between the ceiling tiles and the wall in kitchen by the cookline, 4 live roaches on spice rack near pizza oven, 1 live roach inside of True reach-in cooler and 1 live on the outside of the door of the same cooler, 2 live roaches underneath True reach-in cooler by the compressor, 6 live roaches underneath ice cream chest freezer, 1 live behind dishmachine, 1 live roach on prep table in kitchen by expo station, 1 live roach behind unused ice machine in storage room between kitchen and bathrooms, 1 live roach on rafter above ice machine.. **Repeat Violation**
  • Stop Sale issued on potentially hazardous (time/temperature control for safety) food due to temperature abuse. All potentially hazardous (time/temperature control for safety) foods in reach-in cooler cold held at greater than 41 degrees Fahrenheit. They were previously shut down in early February of this year and in May of last year, both times for roaches. We try not to be judgmental in these reports, but we wouldn’t eat at this place if you paid us.

The biggest number in our report this week was for China Gardens, on Military Trail in Delray Beach just north of Atlantic. They had 21 violations including 6 marked high priority:

  • Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Coming in from outside and start food prep. **Corrected On-Site**

Cut cabbage 60°, bean sprouts 61° on counter on cookline. Person in Charge placed items in walk-in cooler. Cooked pork 61°, cooked noodles 60° at expo line. Person in Charge placed items over ice. **Corrected On-Site**
  • Raw animal foods not properly separated from one another based upon minimum required cooking temperature in walk-in cooler. Raw chicken stored over raw pork in walk-in cooler. Person in Charge repositioned chicken to lowest shelf. **Corrected On-Site**

In East Boca, Arturo’s on Federal Highway had 14 violations including 6 marked high priority and a stop sale on marinara sauce:

Potentially hazardous (time/temperature control for safety) food cooked/heated yesterday/on a previous day not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within a total of 6 hours. Food has been cooling overnight. See stop sale. Marinara sauce in walk-in cooler 41°-48° cooling overnight. Person in Charge discarded sauce.

Others with somewhat high numbers include:

  • Far East Wok on Sample in Pompano, with 20 violations, but only 1 marked high priority.
  • Creative Catering on Congress in Delray (17, 3)
  • China & Japan Express, Deerfield (13, 1)
  • Taqueria Huetamo in Delray improved on their rodent dropping inspection in late January, this time with 13 violations, only 1 high priority.
  • Lucky Express on Clint Moore in Boca (11, 0)
  • Great Wall on Lyons in Coconut Creek (11, 1)
  • Village Cafe, Oriole Blvd in Delray (10, 3)

On the good news side of things, our friends at Steve’s Wood Fired Pizza (Boca Lyons Plaza at Lyons and Glades) had a perfect inspection. It’s marked as a call-back in the spreadsheet but as a routine inspection on the licensing portal. Steve’s has consistently excellent inspections.
Paneterie on Jog in Delray (just north of Clint Moore) also had a perfect inspection.
Boston Market on Beracasa Way had only 1 violation.
Burger King in West Boca Square (the Target plaza on 441) had 2 minor violations.
Golden Corral on Congress in Delray had only 2 minor violations. That’s a very good showing for a buffet restaurant.
Cannoli Kitchen on Powerline in West Boca (near Olive Garden) had 2 minor violations. Tijuana Flats in the same plaza had 4 violations. Hong Kong Palace had 8 violations.
HotDog-Opolis on Glades near the Turnpike had 5 minor violations.
Chow Thai in Sandalfoot had 8 violations (1 high priority). Pei Wei on Beracasa also had 8 violations (3).
Puccini’s Pizzeria at Yamato and 441 had a licensing inspection with 8 violations including 2 high priority.

Cucina Fra Diavolo Shut Down Again for Roaches: Restaurant Inspection Report

Inspectors shut down Delray’s Cucina Fra Diavolo after finding roaches:

Roach activity present as evidenced by live roaches found…..3 live roaches in shelves ,2 live roaches underneath shelf by the ice cream freezer , 2 live roaches inside door gaskets in ice cream freezer , 5 roaches egg cases between shelf in kitchen prep area.

There were a total of 23 violations including 6 marked high priority:

Live, small flying insects in kitchen, food preparation area, or food storage area. ….live small flying in kitchen prep area and station in front line.
Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit……butter 70°/71° in server station , some butter being held more than 4 hours ( stop sale issue) operator discarded it. , some are 1 hours ( time marked butter )

They reopened the following day after a follow-up inspection. The same restaurant was also shut down for roaches in May of last year. Cucina Fra Diavolo is on Military Trail just north of Atlantic.

Also in Delray (on Atlantic west of Jog), Ho Ho Chinese had another bad inspection. This time they had 19 violations including 3 marked high priority. In December they had 28 violations with 4 high priority.
It wasn’t all bad news. We noticed good inspections for several restaurants. Domino’s Pizza on Powerline wins this report after a perfect inspection with zero violations. Others with good inspections include:

  • Chipotle (Glades & Butts) – 1 minor violation
  • The Wick (Federal Highway in East Boca) – 1 violation
  • Cheri’s NY Top Dogs (food truck) – 2 minor violations

Other notable inspections include:

  • Terra Fiamma (Delray Marketplace) – 14 violations, 4 high priority
  • El Balcon de las Americas (Powerline in Deerfield) – 14, 7
  • Hooters (NW 19th, just east of Town Center) – 10, 2
  • Rancheritos de Boca (Somerset Shoppes – Lyons at Glades) – 10, 2
  • Bamboo Wok (Coconut Creek) – 7, 1
  • Gyroville (near Hooters) – 7, 2
  • Pollo Tropical (Military Trail in Deerfield) – 7, 0
  • Romano’s Macaroni Grill (Military Trail north of Glades) – 5, 1

Also we saw four licensing inspections. The new Kanpai (Palmetto Park & Federal Highway) had five violations including one high priority, which is not great for a licensing inspection. We’ve been looking forward to this one and will try it.
The other three were perfect: Paladar Colombiano (Federal Highway in Boca); Talia’s Tuscan Market (NE 2nd Street in East Boca); and Hotdogopolis at 6020 N Federal Highway (Boca).
