Inspections: The Good, the Bad, and the Roaches

Souvlaki Fresh leads off our latest restaurant inspection report, with a temporary closure on June 27th due to roaches. The number of violations wasn’t that big, but sometimes the lack of quality outweighs the quantity.

Dead roaches on premises….5 dead roaches underneath POS by the reach in cooler in front line. 1 dead roach underneath shelf , 1 dead roach by the reach in freezer , 2 dead roaches by the mop sink in back prep room.
Pesticide/insecticide labeled for household use only present in establishment….RAID ants and roach killer spray in frontline underneath POS.
Potentially hazardous (time/temperature control for safety) food cooked/heated yesterday/on a previous day not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within a total of 6 hours. Food has been cooling overnight. ….slice gyro 46°/49° in glass door reach in cooler. See stop sale. Operator discarded food. **Corrective Action Taken**
Raw animal foods not properly separated from one another based upon minimum required cooking temperature in reach-in cooler….raw chicken above raw beef, raw fish in reach in cooler. **Corrected On-Site**
Roach activity present as evidenced by live roaches found….2 live roaches on floor mat by the reach in cooler in front line kitchen. 10 live roaches underneath 2 compartment sink , 1 live roach con the wall between reach in freezer and 3 compartment sink , 1 live roach on the floor in back prep room.

Souvlaki Fresh is in the plaza just south of Mizner Park. They had a much better inspection the next day, but July 12th inspectors again found dead roaches. A follow-up on the 13th showed zero violations.

The biggest number in our report goes to China Buffet & Grill in Deerfield Beach (Hillsboro west of Powerline). Inspectors noted 34 total violations on July 11th, including 13 marked high priority. This place is an ongoing inspection disaster, featured in our late May report with 40 violations and a 2-day closure, and then a week later they were closed again and were caught “dumpster diving” on video.
Get ready for this list of high priority violations:

  • All potentially hazardous (time/temperature control for safety) foods on buffet cold held at greater than 41 degrees Fahrenheit. Observed imitation crab salad (51°F); coleslaw (51°F); pasta salad (49°F); cut tomatoes salad (55°F); cut lettuce (58°F); on the salad buffet.
  • Cooked meats or poultry hot held at less than 135 degrees Fahrenheit or above. Observed cooked chicken 84° stored in the kitchen on the fry station at room temperature
  • Dented/rusted cans present. See stop sale. Observed 4 dented cans on the can rack. **Repeat Violation**
  • Employee failed to wash hands before changing gloves and/or putting on gloves to work with food. The cooks **Corrected On-Site** **Repeat Violation**
  • Employee handled soiled dishes or utensils and then handled clean dishes or utensils without washing hands. The dish washer. **Corrected On-Site**
  • Employee touching ready-to-eat food with their bare hands – food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed the dish washer grab cooked shrimp with bare hands from a container filled with cooked shrimp to eat the n the kitchen. **Corrected On-Site**
  • Employee washed hands with no soap. The sushi cook.
  • Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Observed cut honeydew 53°F and cut cantaloupe 53°F on the sushi and fruit side of the buffet. **Repeat Violation**
  • Potentially hazardous (time/temperature control for safety) food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. The sushi rice. Rice still hot cook just add the vinegar. **Corrected On-Site**
  • Raw animal foods not properly separated from one another based upon minimum required cooking temperature in walk-in cooler. Observed raw chicken stored over raw shrimp, raw blue crab, and raw ground pork in the walk-in cooler. **Corrected On-Site** **Repeat Violation**
  • Raw animal foods not properly separated from one another based upon minimum required cooking temperature in walk-in freezer – all products not commercially packaged. Observed raw chicken stored over raw beef in the walk-in freezer. **Corrected On-Site**
  • Single-use gloves not changed as needed after changing tasks or when damaged or soiled. The cooks **Corrected On-Site**
  • Stop Sale issued on potentially hazardous (time/temperature control for safety) food due to temperature abuse. Follow-up inspections on July 18th were much better.

The other big number in this report belongs to the Sushi Thai restaurant in the same plaza as Souvlaki Fresh, just south of Mizner Park. Inspectors found 27 total violations on July 8th, including 6 high priority. Mostly they were related to hand washing and storage of raw animal foods.
Others with more than 10 violations include: Duffy’s on St. Andrews (19 violations, 2 high priority); Chen’s Yokohama in Boca Lyons Plaza (16, 5); New Golden China on Palmetto near Powerline (14, 3); and Grille on Congress (13, 3).

On the good news side of things, the Subway on Powerline (near Olive Garden) had a perfect licensing inspection in this report. A few weeks later they had 4 violations, 2 high priority which is still pretty good.
Near-perfect inspections were reported for:

  • Bakery of France on SW 18th (1 violation, 1 high priority)
  • Four Seasons on Glades near Boca Rio (2, 0)
  • Einstein Bagels in Shadowood (2, 1)
  • Pizza Hut on 441 near SW 18th (3, 0)

Other notables in this report include:

  • Rotelli’s in its new location near Hobby Lobby (4, 1)
  • Casa Tequila on Glades across from the International Jewelry Exchange (4, 0)
  • Houston’s near Town Center (4, 3)
  • Sicilian Oven on St. Andrews (4, 0)
  • Tijuana Flats in Mission Bay (4, 0)
  • Padrino’s in Mission Bay (5, 0)
  • Dominic’s on Glades near Boca Rio (5, 1)
  • Puccini’s on Yamato (5, 1)
  • Dominic’s in Westwinds of Boca (9, 2)
  • Bamboo Wok on Glades near Lyons (9, 1)
  • First Watch on St. Andrews (10, 2)
  • Another Broken Egg in Royal Palm Place (10, 3)

One last note, China Cafe in Logger’s Run reopened with a so-so licensing inspection, 8 violations, 1 marked high priority.
